The Eliminating Health Disparity Initiative (EHDI) is a grant program within the Center for Health Equity. EHDI was established during the 2001 legislative session. The program invests about $5 million dollars annually in community initiatives that address health inequities for populations of color and American Indians across eight different priority health areas.

Project name: EHDI-White Earth Band of Chippewa

Project Description: The White Earth Band of Chippewa Indians will utilize the TANF and State General Funds in the Eliminating Health Disparities Initiative to provide Health Promotion/Direct Services to our target population to address the following priority health areas: 1). Increasing prevention and education services to address unintentional injuries caused by violence and home fires by providing culturally responsive anger management/domestic violence prevention services and fire safety education/ training; 2). Providing nurse home visiting services to increase prenatal care by reducing barriers to health care and prenatal/infant/early childhood education and support services; 3). Improving access to immunizations for adults and children; and 4). Increasing protective factors related to teen pregnancy prevention and health education/services through school nursing.

EDHI Grant Activity Outcomes: 1) Increase prevention and education services delivered to the target population to address unintentional injuries caused by violence and home fires. 2) Provide nurse home visiting services to increase prenatal care for 75% of program participants by reducing barriers to nursing care and prenatal/infant & early childhood education and support services. 3) Improve access by 10% to immunizations for adults and children living on or near the White Earth Reservation by June 2025. 4) Protective factors related to teen pregnancy prevention and health education/services will increase through school nursing services at contracted schools.
