The Eliminating Health Disparity Initiative (EHDI) is a grant program within the Center for Health Equity. EHDI was established during the 2001 legislative session. The program invests about $5 million dollars annually in community initiatives that address health inequities for populations of color and American Indians across eight different priority health areas.

The ANIKA Foundation

Project name: Quality Life MN

Project Description: The ANIKA Foundation proposes the Quality Life MN campaign - a holistic wellness approach to eliminating health disparities in the African American community across cardiovascular diseases, breast & cervical cancers, diabetes, HIV/AIDS and Teen Pregnancy. We'll provide health promotion & direct services to 2,000+ people per year in key population centers in Minnesota, reaching 8 million Minnesotans through a mass media and messaging campaign with grassroots community engagement. Annual direct services, include connecting 250 African American Women with mammograms, 1,000 blood pressure and 500 blood sugar checks, connections to local healthcare providers and insurance access; 250 Teens with HIV/AIDS & STI education and pregnancy prevention. We also drive institutional change, address SDOT by developing culturally specific community health curriculum delivered by trained CHWs. These strategies are in collaboration with key partnerships rooted in community.

EDHI Grant Activity Outcomes: 1) Reduce cardiovascular disease rates, diabetes, breast & cervical cancer rates in Black and Native American communities by increasing awareness and resource connection. 2) Increase access to free/low-cost screenings. 3) Expand the Cancer, Diabetes, Heart Disease workforce to include Community Health Workers (CHWs) and Navigators. 4) Address root causes through case management, wrap around support, impact of systemic racism, resource connection.
