The Eliminating Health Disparity Initiative (EHDI) is a grant program within the Center for Health Equity. EHDI was established during the 2001 legislative session. The program invests about $5 million dollars annually in community initiatives that address health inequities for populations of color and American Indians across eight different priority health areas.

Southside Community Health Services

Project name: Southside Community Breast and Cervical Cancer Screening Pr

Project Description: Southside Community Health Services (Southside) is a 501 (c)(3) nonprofit, Federally Qualified Health Center providing access to primary and preventive care for un- and under-insured residents of South Minneapolis and the surrounding areas. The EHDI project targets breast and cervical cancer screening disparities among Hispanics/Latino and African American women (all people who have a cervix). The EHDI project addresses the first level of change, with activities expanding screening access and provide health education about screening in South Minneapolis. The project addresses the second level of change, with activities focused on improving Southside’s internal systems. The EHDI project will have eight staff members: a Women’s Health Provider (MD), a Women’s Health Nurse Practitioner, a Women’s Health RN, a mammography technician, a SAGE Coordinator, a Cancer Care Coordinator, a Quality Director, and a volunteer OB/GYN. Evaluation will be a key element at every stage of the project.

EDHI Grant Activity Outcomes: 1) Deliver a health promotion and prevention project grounded in cultural knowledge and wisdom that contributes to eliminating disparities through direct service. 2) Increase the number of Hispanic/Latina and African American women in South Minneapolis receiving annual breast and cervical cancer screening at Southside from 644 to 2,707, and 3) Participate in and leading efforts that contribute to eliminating disparities by changing organizational policies and changing the way a system in an organization works in order to improve health.
